Directions for completing this form
Advance Registration is advised and must be received by September 6, 2004. After September 6, the regular fees apply to all registrants. Phone registrations are not accepted and full payment must accompany all registrations.

**CANS members receive a $50 discount. Log on to www.nursingscience.org to renew your membership or to become a CANS member or call Rebecca Gade at 414.287.0289 for more information.

Registration forms submitted via mail or fax must be day/time stamped by 11:59 p.m., September 6, to qualify for advance registration and to receive discounted rates.

What to Wear
Appropriate dress for the conference is business casual. Please bring a jacket or sweater as temperatures in meeting rooms may vary.

Cancellation Policy
Cancellations must be received in writing by Friday, September 17, 2004. After this date, refunds will not be issued. To receive a full refund, less a $75 processing fee please send written requests by September 17 to:
